Memorial Hermann Advantage HMO is provided by Memorial Hermann Health Plan, Inc., a Medicare Advantage organization with a Medicare contract. Enrollment in this plan depends on contract renewal. Medicare beneficiaries may also enroll in Memorial Hermann Medicare Advantage Plans through the CMS Medicare Online Enrollment Center located at www.medicare.gov.
The Memorial Hermann Dual Advantage HMO D-SNP is a Dual Eligible Special Needs Plan (D-SNP). Ability to enroll in this plan will be based on verification that enrollee is entitled to both Medicare and medical assistance from a state plan under Medicaid.
