You are eligible to enroll in or switch to a Memorial Hermann Medicare Advantage Plan if you meet the following requirements:
- You are currently enrolled in Original Medicare (Medicare Part A and Part B)
- You live within Harris, Fort Bend, Montgomery, Brazoria, Hardin, Jefferson, Liberty, or Galveston counties in Texas
- You don’t have disqualifying medical conditions.
- If you have both Medicare and Medicaid coverage and live within Harris, Fort Bend or Montgomery Counties, you may be eligible for the Memorial Hermann Dual Advantage Special Needs Plan (D-SNP). To learn more, visit the D-SNP section of our website.
