Helpful Tips for Navigating Memorial Hermann Health Plan Flexible Spending Program (WEX) Card
Please read these helpful tips and download our Memorial Hermann Flexible Spending Program (WEX) Card App before you begin using the card, this will provide the best member experience and make the most of the benefits.
- When you receive your card, the card is ready for use. You do not need to set up a PIN; however, if you would like to setup a PIN, call (833) 647-9633
- When making a purchase, process the card as CREDIT, not FSA/HSA or debit.
- Your Memorial Hermann Health Plan Flexible Spending Program (WEX) card can only be used at primary IN-NETWORK stores.
- Primary in-network stories:
- Kroger
- Randalls
- CVS
- Walgreens
- Walmart
- This network currently does not include HEB or Amazon
- Please download the app, Memorial Hermann Flex Card (link below), which allows you to scan products right in the store to determine if they are covered.
- Helpful Tip! Remember to scan the UPC on the ITEM, not the UPC that may be displayed on the store shelf.
- When ordering OTC items from the Medline Catalog, pay with your Memorial Hermann Flexible Spending Program (WEX) card (again, process as a credit card).

Reimbursements
If you have eligible out-of-pocket expenses for reimbursement please click here for the Reimbursement form. You must follow the step by step instructions on the request form before submission. Please be advised that missing information may result in the denial or delay of your request. This form is for new claims only. Reimbursements can also be submitted through the app, Memorial Hermann Flex Card on your mobile device. Paper reimbursement forms should be mailed to Memorial Hermann Health Plan, P.O. Box 2623, Fargo, ND 58108-2623. If you have any questions about completing this form, please contact Memorial Hermann Flex Card Services at (833) 504-1839 (TTY: 711).
Each Memorial Hermann Advantage Health Plan member has a Memorial Hermann Flexible Spending Program (WEX) card. Check your Summary of Benefits document to get individual plan amount details. All plans have a Memorial Hermann Flexible Spending Program (WEX) card to use on the following:

- Over-the-Counter (OTC) products can be purchased via the Medline catalog or by using the card at primary IN-NETWORK stores.

When ordering from the Medline catalog, be sure to check the OTC balance on your Flex Card first, because you can only spend up to your balance. You will not be able to exceed your available OTC balance and use a second form of payment to cover the difference.
