The Memorial Hermann Flex Card is a new benefit for the plan year 2024. Members of Memorial Hermann Advantage Plans will receive their personal flex card at the beginning of 2024.

Your Flex card will automatically activate at the start of 2024.

Members can visit the Flex card portal beginning January 1, 2024 to see the balance on their card and complete other actions, such as submitting a reimbursement request if they forgot to use the card.

Helpful Tips for Navigating Your Flexible Spending Card

Please read these helpful tips and download our Flex Card App before you begin using your card. It will improve your experience and ensure you get the most from your benefit.

  • When you receive your card, it is ready to use. You do not need to set up a PIN. When making a purchase, run the card as CREDIT not FSA/HSA or debit.
  • At this time, please avoid purchasing Vision or Hearing products at Walmart. You can still purchase OTC products at these locations (again, please pay at a cashier lane, not self-checkout).
  • Your flex card can only be used at IN-NETWORK stores. This network currently does not include HEB or Amazon.
  • Primary in-network options:
    • Kroger
    • Randalls
    • CVS
    • Walgreens
    • Walmart
  • Please download our Flex Card App (link below), which allows you to scan products right in the store to determine if they are covered. Remember to scan the UPC on the ITEM, not the UPC that may be displayed on the store shelf. Very helpful!
  • When ordering OTC items from the Medline Catalog, remember to pay with your flex card (again, use it like a credit card). If you are experiencing issues with Medline ordering, please call Medline directly (not Memorial Hermann Customer Service) – as they will be able to assist you. The Medline number is: (833) 511-9844.

If you have eligible out-of-pocket expenses for reimbursement please click here for the Reimbursement form. You must follow the step by step instructions on the request form before submission. Please be advised that missing information may result in the denial or delay of your request. This form is for new claims only. Reimbursements can also be submitted through the Flex Card app on your mobile device. Paper reimbursement forms should be mailed to Memorial Hermann Health Plan, P.O. Box 2623, Fargo, ND 58108-2623. Each Memorial Hermann plan will have a flexible spending card benefit for 2024. Check your Summary of Benefits document to get individual plan amount details. All plans have a Flex card benefit to use on the following:

Over-the-Counter (OTC) products can be purchased via the Medline catalog or by using the card at approved brick and mortar locations. Below are a few locations where your flex card is valid (please note that HEB is not a participating location):

  • Albertsons
  • CVS
  • Kroger
  • Walgreens
  • Walmart Super Centers

When ordering from the Medline catalog, be sure to check the OTC balance on your Flex Card first, because you can only spend up to your balance. You will not be able to exceed your available OTC balance and use a second form of payment to cover the difference. To check your available balance go to to access your Flex Card portal.

Members of the Memorial Hermann Dual Special Needs Plans (DSNP) for people with both Medicare and Medicaid will also receive a Grocery Benefit on their Flex card.

Members on other plans who qualify, enroll and complete a Case Management program may also be eligible to receive a Grocery Benefit added to their Flex card. For a more detailed explanation of the grocery benefit, click here.
