Applying for a Memorial Hermann Advantage HMO Plan Is Easy, Secure and Convenient!

To be eligible to elect a Medicare Advantage plan, like Memorial Hermann Advantage HMO, an individual must be entitled to Medicare Part A and enrolled in Part B, and must be entitled to Medicare Part A and Part B benefits as of the effective date of coverage under the plan. To enroll in a Memorial Hermann Advantage HMO plan, you must also reside in Harris, Brazoria, Fort Bend, Hardin, Montgomery, Liberty, Galveston or Jefferson county in Texas.

Once you have confirmed you are enrolled in Medicare and are within a valid election period, you can enroll in a Medicare Advantage Plan.

Enroll Online

You can enroll in a Memorial Hermann Advantage HMO plan at your convenience by visiting our secure and easy online enrollment site. Medicare beneficiaries may also enroll in a Memorial Hermann Advantage HMO plan through CMS Medicare Online Enrollment Center located at medicare.gov. (Please note that by clicking on this link, you will be leaving Memorial Hermann Advantage website.)

You will first be asked to enter your ZIP code to confirm you live within the eligible area of our Medicare Advantage plans. Then, our online enrollment tool will allow you to view our Memorial Hermann Advantage HMO plans, cost, coverage, benefits and additional plan details.

Call one of our licensed Medicare agents at (866) 350-2715 (TTY 711) from 8 a.m. to 8 p.m. CT, 7 days a week from Oct. 1 – March 31 and 8 a.m. to 8 p.m. CT, Monday – Friday from April 1 to Sep. 30. What Happens After I Enroll?

Once you have submitted your enrollment information via phone, online or mail, Memorial Hermann Advantage HMO will securely send your application to the Centers for Medicare & Medicaid Services (CMS) for approval. Upon approval, you will receive a Memorial Hermann Advantage HMO Membership Welcome Kit packet, along with your new ID card granting you access to numerous doctors and hospitals, your specific plan benefits, and more. You can typically expect your new ID card and membership packet within 10 business days of your confirmed enrollment from CMS.
