Memorial Hermann Advantage HMO members may now request to receive a printed Provider and/or Pharmacy Directory, as well as a complete Formulary document in the mail. Please note that our Printed Provider directory is over 1,000 pages long, so to conserve paper, we encourage you to download a digital directory here.
Simply complete the form below and your requested copy or copies will be mailed to you shortly. In the meantime, you may also search for your Provider, Pharmacy and Covered Drugs (Formulary Search) online.
